| Visitors of our website have two options for obtaining our
charge account application. You can view, save, and print our credit
application in the Adobe Acrobat Reader format, or click on the links below to
view and print a copy from your web browser. Printing the application can be
done by either clicking on the "Print" icon in your browser's
toolbar, or click "File" then "Print" in the menu bar.
After completing and signing the application, you may submit it to us via U.S.
Mail, or by facsimile at (408) 292-6625. The credit application, as well as the
addendum, must be completed, signed, and submitted in order to obtain credit
account consideration. Average credit account approval is approximately 2 to 5
